Read below steps carefully and follow every step in adobe Photoshop, you can take video help also which is given end of the steps.

• Open adobe Photoshop, and after open Photoshop Open your Photo in Photoshop (File menu-Open)
• Press "Ctrl+a" , "Ctrl+c" and "Ctrl+w" (Select, Copy and close)
• Now press "Ctrl+n" for new document. set size "width=851 pixels", "Height=315 pixels" and "Resolution=72 pixels" and Press OK.
• Now go to "View Menu - New guide" - a new window will be open, select "Vertical" "Position=20 px" and press OK.
• Repeat above step again and set "Position=180px" and press OK.
• Repeat above step atain and select "Horizontal" "Position=175 px" and Press OK.
• Now Press "Ctrl+v" for your copied image Paste here and use "Ctrl+t" for free Transform. (adjust your image cover and profile pic. you can see left and bottom guide box, that is profile pic. box) and press Enter.
• Now go to layer menu and select "Flatten Image"
• Now select "Rectangular marquee tool" From Tool bar. fix "rectangular marquee tool" size ▬ from Options bar - select "style=fixed=size" and set "width=160 px" Height=160 px"  
• Now select profile pic., with "Rectangular marquee tool" and press "Ctrl+c" for copy.
• Press "Ctrl+N" and take new document with size "width=160 pixels" "height=160 pixels" "Resolution=72" and press OK.
• and Paste (Ctrl+v) here, and "go to Layer menu-flatten image" , next "go to Image menu- Image size" new window will be open set size here width=180 px, and Height=180 px. press OK. and save this image.your profile pic. , in .jpg format.   
• Now Select Previous Document and "go to Image menu-Canvas size" set width=851px, and Height=315px, "anchor=upside arrow" (select any upside arrow). clik OK. 
• Save this image as cover photo in .jpg format. and use on Facebook as cover photo.

Abstract : 

Take new Document in adobe Photoshop with the size of 851x337 pixels and set guide line vertical 20px and 180px and horizontal 175px, and now set here your photo and save profile pic. and cover pic.
